Service-learning courses offered: Courses that have a service-learning component are indicated on the course schedule.

Student waiver for service-learning
All students participating in service-learning courses should fill out and sign this waiver and return the signed waiver to their instructor or to the Coordinator of Experiential Learning. You will need to indicate the course you are taking and the name of the service-learning placement in the space reserved for this information on the form. If you are under 18 years of age you must also have your parent or guardian sign the form.
Time Sheet for Service-Learning Placements
Use this form to keep track of the time that you spend at the service-learning site. You should have the site supervisor sign the form each time you work. Keep this form in a safe place. Your instructor may ask you to submit this at the end of the semester.
