Internship Learning Agreement
In order to register for an internship, students should fill out the Internship Learning Agreement below. Part of the form can be filled out by the student alone, however the sections on learning objectives and job description should be filled out with the student’s supervisor. The student’s advisor or the instructor overseeing the internship should fill out the portion regarding academic requirements. These requirements may include keeping a journal, writing a paper, or performing other reflective written assignments, as directed by their academic advisor and or supervisor. These assignments are not necessarily the student’s only obligation in order to obtain academic credit.
Internship Waiver
All students doing an internship off campus should fill out and sign the waiver below before beginning their internship. Students who are under the age of 18 should also have their parent or guardian sign the waiver. Once it has been filled out and signed it should be returned to the Coordinator of Experiential Learning, room 108 Brown Science Building.
Internship Evaluation Forms
During the internship, the Coordinator of Experiential Learning will contact the internship supervisor and the student. During the last two week of the internship, where possible, the Coordinator of Experiential Learning will make a site visit to meet with the internship supervisor and the student to evaluate the internship experience. Both the student and the employer should fill out a written evaluation as well. The evaluation forms can be found below.
